All new service providers are subject to a $1,000 one time setup fee. This fee is currently being waived from March-September of 2021 as part of a special promotion.
This setup fee covers all application onboarding activities including; initial student/parent data importation, staff training, digitization of unique forms, and ongoing application support.
After launch, FormConnect provides 4 hours of form creation, training and support every 3 months. Additional hours of support are charged at $20/hour.
* Service providers have 45 days from the invoice issuance date to provide payment to FormConnect.
Failure to do so will result in your FormConnect account being suspended until payment is received.
